Error During Installation No Database Received

I can not install the program is correctly installed and located in the root folder of my website as shown in the image.
It seems there is a problem on your server which is not providing the database for the installation.
Check the php and firewall settings and permissions and everything is correct from my side.
the database in our system is created clean and with the user with all permissions
