You can as well set your $config['log_threshold']  to 1 in the config.php file line 214, filepath: app\config\config.php, then repeat the action and see if it captures any error in the app\logs folder that might help in knowing the cause.

Thizzi POS I managed to resolve this by installing mod_security and changing this line in .htaccess from:

RewriteRule ^(.*)$ index.php?/$1 [L]

To:

RewriteRule ^(.*)$ index.php?/$1 [B,L]
